Driver's license: Apply for a copy of the file card
If you need a replacement driver's license after moving to another municipality, the new driving license authority may request a copy of your driving record card.

Service description
If you have moved to another municipality and need a replacement driver's license there (e.g., if you have lost your driver's license or are exchanging your old driver's license for an EU card driver's license), the responsible driver's license authority may require you to provide a card index transcript.
The card index transcript is a certificate from the local driver's license register containing your personal driver's license data.

We will then send the index card transcript directly to this authority.
If you have moved your place of residence to a member state of the European Union and need an index card transcript there to deal with your driver's license matters, please send us an informal letter and enclose a copy of your identity document. The index card transcript will be sent to the relevant foreign authority via the Federal Motor Transport Authority.
IMPORTANT NOTE: If your old paper driver's license (gray or pink) was issued by the state capital of Mainz before 1996, but you were residing in the Mainz-Bingen district at that time, this data is no longer available in Mainz. In this case, please contact the Mainz-Bingen district administration or the driver's license authorities (driver's license offices) in Bingen or Oppenheim directly.

Please also note that due to the time required to accept an affidavit in the event of a driver's license loss, we can only accept these on Thursdays until 5 p.m.
